PLEASE SUBMIT THIS SURVEY TO THE ASAA OFFICE NO LATER THAN MAY 23, 2008.

It is very important that the instructions contained on the form be read before submitting and completing the form. Where appropriate, indicate whether the sport includes combined teams, or girls on boys teams or vice versa. If even one (1) boy is identified as participating on a girls' team, please record the figure and note it by a doubled asterisk (**). The same applies to girls playing on boys' teams.

NOTE: "Competitive Spirit Squads" is listed on the Sport Participation Survey. Please record the participation numbers on this Sport Participation Survey for any spirit squad which participates in a regional and/or ASAA State championship. (Examples of competitive spirit squads are: cheerleaders, stunt squads, drill team, pom and dance teams).

Spirit Squads that are non-competitive i.e., participants who do not compete in a regional and/or ASAA State championship must be listed on the Actitivities Participation Survey.

We have asked your school district superintendent to verify with you that you have SUBMITTED this survey. We are requesting that the surveys be submitted to our office no later than May 23, 2008.
